Targeted Care Coordination
Who It's For:
Adults and children with mental health challenges who need help coordinating multiple services. Covered by Texas Medicaid.
The Problem
You're dealing with mental health challenges AND trying to navigate housing, benefits, schools, doctors, and a dozen other systems that don't talk to each other. It's exhausting.
The Solution
Our case managers become your navigators. We coordinate your care across all the different systems medical, mental health, housing, education, benefits so you don't have to do it alone.

• Comprehensive needs assessment
• Connection to medical, psychiatric, and social services
• Help with benefits applications (Medicaid, SSI, housing assistance)
• Coordination with schools, employers, and healthcare providers
• Regular check-ins and progress monitoring
